Prepare a clear statement about suspected SIM fraud

Organise facts for a complaint or police report without inventing legal sections, accusations or promised outcomes.

Prepared 8 October 2026 · General guidance; check current provider requirements

A useful statement explains what you observed and what action you need. A template is not an FIR, and submitting a description online does not establish that a formal case has been registered.

Put facts in order

Start with the date you noticed the issue, the affected connection and the first signs. Explain any service loss, unauthorised messages or disputed transactions. State what the operator or provider has already told you.

Keep an evidence list

Number your supporting items: the service interruption screenshot, the complaint reference, the message and the relevant transaction record. Keep the originals. Redact unrelated private information only from copies used for non-official sharing.

Make a clear request

Ask the relevant authority to investigate the specific event and advise on protective action. Separate confirmed information from your suspicions. Do not add criminal-law sections copied from an unrelated website.

Get confirmation

Ask how the submission is recorded, how you will receive a reference and how to follow up. A lawyer or the receiving authority can advise on the formal wording and applicable procedure.

General preparation guidance is distinguished from operator-specific requirements. Confirm the requirements for your own transaction with the responsible provider.
